1) Build a website first. And find a reputable web designer. Stress the need for “Search engine optimization” (SEO). This is a way to insure that sites like google and yahoo place your site toward the top of searches under specific key words. It is not cheap to find someone who is good at this. My company charges >10,000 for 5 key words. We are a Business to business designer so no need to advertise myself here.
2) offer a catalog with shopping cart and focus on filling orders quickly.
3) Help to develop a brand name for yourself through news stories, banner ads and tile ads. advertise in sites that are appropriate.

I am looking for some rule-of-thumb information for expectable advertising revenues for a consumer-oriented, medical website. Banner advertising can be quite well-targetted within the site. Advertisers will include pharmaceutical companies, medical product companies, hospitals, other medical facilities, physicians, and health insurers and related health financing companies. I’m looking to get a rule of thumb approximation of expectable advertising revenue per user or per visit perhaps with a low-medium-high range, as different sites will of course be able to charge higher rates than others.

on the flip side, a program like nielsen galaxy adviews can tell you how much competitors are spending for internet advertising.
